In the event of an emergency after regular business hours, please seek immediate assistance by calling 911.

Should you be taken to the emergency room for evaluation, the emergency room doctor will be able to contact your physician who is on call for emergencies 24 hours a day, 7 days per week.

In order for our doctors to provide you with prompt and complete care during your visit, please be sure to bring the following with you:

  • Insurance Information/cards
  • All current medications — prescription bottles preferred

If you are a current patient, please contact your physician’s office and complete the enrollment process to begin using the portal.

Once enrolled, you can review clinical information from your most recent visit.

Prescription Refills

Requests for prescription refills should be made during routine office hours, Monday through Friday, and will generally be handled within 24 — 48 hours.

Refill requests will not be taken after regular business hours. Please request prescription refills 10 days prior to finishing your medication.

Appointment Cancellation Policy

In an effort to have as many appointments available to our patients as possible, it is important to give our staff 24 hours’ notice if you need to cancel or reschedule an appointment.

Clear Creek may charge you a no-show fee of $50 if the 24-hour notice is not provided. Thank you for your cooperation in this matter.
